Medical History – 36 y/o female with pre-diabetes, hypothyroidism, and overweight for the last 3 years.
Goal – To lose weight, reduce fasting blood sugar, and reverse pre-diabetes.

Lifestyle Medicine Management –

  • After working on four pillars of lifestyle medicine, she lost her first 15 pounds in the first 4 months.
  • Her HbA1C reduced from 5.9 mmol/l to 5.4 mmol/l.
  • After working on the regime for the next 4 months, she lost 15 pounds more, and the fasting blood sugar reduced from 5.8 mmol/l to 4.9 mmol/l.

Medical History – 38 y/o female with diabetes x 1 year, sedentary lifestyle, and obesity for the last 10 years.
Goal – To reverse diabetes.

Lifestyle Medicine Management –

  • Followed circadian rhythm intermittent fasting and would finish her dinner by 7 pm.
  • Added 8000-10000 steps daily in her physical activity.
  • Changed her diet to add whole food plant-based diet and reduced intake of animal foods to <10% of calories.
  •  Improved sleep hygiene by getting 7-8 hours of sleep every night.
  • In 12 months, she lost more than 50 pounds from 200 pounds to 147 pounds. At this time, her goal is to maintain her lifestyle changes and achieve her goal weight of 135 pounds.
Case Study 3

Medical History – 72 y/o male with h/o diabetes, hypertension, hyperlipidemia, overweight, sedentary lifestyle, and ankylosing spondylitis. He was diagnosed with gout (uric acid of 9.8 mg/dl) and creatinine of 1.89 mg/dl.

Goal – To stop taking medication for gout and reduce the creatinine level.

Lifestyle Medicine Management –

  • Reduced protein intake from >120 gm to 60 gm daily.
  • Reduced animal protein and increase plant protein intake.
  • Increased fiber intake.
  • In 12 weeks.
  • Uric acid reduced from 9.8 mg/dl to 5.5 mg/dl. Stopped taking medication for gout.
  • Serum creatinine from 1.89 mg/dl to 1.6 mg/dl.
  • Total cholesterol at 169 mg/dl, LDL at 98 mg/dl without any medication for hyperlipidemia. 
Case Study 4

Medical History – 75 y/o female with hyperlipidemia for many years, and overweight. She is on only one prescription
medication, Statin for hyperlipidemia.

Goal – To stop taking statin medication.

Lifestyle Medicine Management –

  • She worked on a small and sustainable goal to adopt lifestyle behaviors for life.
  • Maintained good sleep health of 7-9 hours a day, and no naps in the afternoon.
  • Increased daily steps to 5000 steps a day.
  • Started working on strength training x 2 times a week and some form of daily exercise program.
  • Reduced fat intake from >100 gm to 30-40 gm.
  • Increased the fiber intake to 30-40 gm. 

Results –

  • Lost 1-2 pounds every month.
  • Reduced LDL from 100 mg/dl to 90 mg/dl.
  • She was told by her PCP that she won’t have to take any statin if her LDL reduces to 80 mg/dl and she is determined to achieve that goal.

3. Schedule your 90-minute strategy session for detailed evaluation and creating a plan together for your success.
4. You will receive a customized plan considering your lifestyle, medical history, allergies, food sensitivities, activity level, etc.
5. You will receive a personalized nutrition plan, with 2000+ recipes to choose from, considering your allergies, food sensitivities, likes/dislikes, glycemic index/load, with an eating schedule.
6. 40 Minutes check-in sessions to address barriers, celebrate success, and recommendation as and when needed.
7. Throughout the program, we will continue with the cycle of Assess -Recommend - Tracking of four pillars of lifestyle medicine i.e. Nutrition, Physical Activity, Sleep Hygiene, and Mindfulness Practices
8. Between sessions, you have access to the client portal messaging system and we are available to answer your questions
9. Once your program ends, you will receive a maintenance program to continue with your health journey.
10. Your biometric assessment and progress report can be shared with your doctor at any time during the journey. We can work with your doctor if needed to bring the best possible health that your deserve.
